How do I make an appointment?
Whether your choice is to have a long distance phone or Skype appointment or to schedule a consultation here at our Institute, how to go about this is the same.
Return to the drop down subject heading of Questionnaires. Open the appropriate questionnaire and complete it on our site. Then click the submit button. Your completed questionnaire will be on Dr. Haskell’s desk the same day.
From the details of your answers and comments, and the complexity of your case, he will know how much time is needed for your consultation.
Dr. Haskell will also decide which lab tests need to be completed before your 1st appointment. Please understand that having your lab work available for this first discussion will greatly improve the outcome and clarity of his recommendations.
Within 1 business day of submitting your questionnaire a member of our staff will contact you by phone, informing you of the time Dr. Haskell will need to spend with you and the tests required, and their costs.
